[MR2] MK2 Turbo Speedometer problem

This problem started when the speedometer stopped working. My first test was to un-plug the speedometer cable from the transmission. I used my drill to drive the speedometer cable to check that component. The speedometer responded which indicated that the cable was ok.
My next step was to try to move the end of the drive mechanism. I found that I could turn the drive mechanism without resistance. This was not a good indication. I removed the right angle adapter and the part that held the speedometer drive gear. I found that the pin between the adapter and the drive gear had bend. This bending also destroyed the drive gear.
I figured that somebody most likely did not correctly install the pin between the adapter and the drive gear. This may not have been a correct assumption.
After installing a new drive gear and pin I checked out the system. I had the car on rear jack stands and let the rear wheels spin. the speedometer indicated the correct speed so I figured I had fixed the problem.
Well I was wrong. When my son went to drive the car he reported the speedometer was again not working. I made a quick check; pulled the speedometer cable and found I could again freely turn the pin in the drive adapter connected to the transmission.
Anybody had a similar problem, solutions are requested?
My first guess is that the right angle adapter may be binding and causing the damage to the drive gear.
